Identify the senior-service categories offered, typical projects or care provided, populations served and any services that are not available.
List the counties and communities served, whether services are provided in person or remotely and any travel limitations.
Provide credentials relevant to the service, licensing information where applicable and evidence of current business or professional insurance.
Describe years in business, experience serving older adults and families, professional affiliations and references that may be contacted during review.
Applicants must provide accurate information, communicate clearly, use fair business practices, maintain required credentials and avoid claiming that acceptance guarantees referrals or endorsement.
Senior Needs Guide may verify information, request additional documentation, limit categories or geographic areas and decline or remove providers. The actual application form will be activated after the provider-review and email-delivery workflow is finalized.
